Each provider’s profile has a message form. You enter your email address and, if you choose, your name, phone number, insurance and a message, and agree to the Terms. The form sends your message to that provider. Submitting an inquiry to a listed provider does not, by itself, establish a clinical relationship with that provider. Users should avoid submitting detailed medical histories, diagnoses, or other unnecessary sensitive information through initial contact forms.
